I think you need a significant amount... but its not an all or nothing system. Your body will fight it as long as it can, but if you notice your breathing getting worse, or harder, as you do more labor (under exertion) then there definitely IS damage done.

The hard part of the diagnosis is... its frequent for laborers to smoke.... so which is causing more damage? If you quit smoking, your lungs will generally heal over time (not fully, but a significant amount can) isocyanate damage WILL NOT heal.
