Fixing Cracks in leather seats?

Hey, well i have cleaned my car up to where it almost looks brand new. Inside and out.

The only problem is that the leather front seats are cracked up from wear and tear. I would love to know how i can fix the cracks and get them to almost brand new. Any sites with this stuff? or any brand names or products i can grab at the store?>

Thanks very much!
